  • M99 Biomechanical Lesions, NEC - Codes for this subsection do not correspond with any categories in ICD-9-CM. Conditions coded include subluxation complex, somatic dysfunction stenosis of the neural canal, and more.

General Guidelines

  • When multiple bones, joints, or muscles are involved, it is appropriate to code for "multiple sites."

  • When coding for pyogenic arthritis, the coder should further classify with the fourth-character specification for the organism, such as staphylococcal, streptococcal, etc., the fifth character provides information regarding the involved joint, and the sixth character provides the laterality for paired joints.

  • Dorsopathies and spondylopathies are specified by type with the fourth character, and the fifth and sixth characters identify the region of the spine affected.

  • When reporting spondylopathies in category M48, a seventh character is required to identify the encounter for fracture treatment.

  • If a patient has infective myositis, interstitial myositis, or a foreign body of the soft muscle tissue, a secondary code is needed to identify the infectious agent or foreign body.

  •  If osteomecrosis is due to a drug or chemical, the coder should first assign a code from T36-T50 to identify that agent, followed by a code from M87.1 to report the condition by body part.

Diseases of the Genitourinary System (Chapter 14)

The genitourinary system includes the urinary bladder, kidneys, kidney tubules, and genitals. Toxins are filtered by the glomerulus, which is inside the kidney. The coder should understand the many anatomic structures and functioning of the genitourinary system. Chapter 14 of ICD-10-CM is titled "Disease of the Genitourinary System," which includes categories N00-N99.

  •  N00-N08 Glomerular Disease - Codes for this subsection correspond with those in categories 580-629 of ICD-9-CM. Conditions coded include disorders of the kidneys and nephrons which contain glomerular filtration units.

Conditions coded include postoperative vaginal adhesions, postprocedural urethral stricture, urinary tract stoma complications, and more.

General Guidelines

  • When a patient has hematuria that is not related to a specific condition, the appropriate code is R31.9, but if the hematuria is associated with a glomerular lesion, the code assignment would be N02.0, Recurrent and persistent hematuria with minor glomerular abnormality.

  • If a patient has urinary incontinence, a secondary code from N39.3-N39.4 should be used to indicate the underlying condition.

  • For a patient with a neurogenic bladder with stress incontinence, the first-listed code would be N31.9, Neuromuscular dysfunction of bladder, followed by a N39.9, Stress incontinence.

  • When coding for conditions in categories N70-N73, the infectious agent should be reported with a secondary code from the B95-B97 range.
